Most fallen Jedi in Legends lose a duel. Ulic Qel-Droma lost the ability to hold a lightsaber at all, and that punishment is more interesting than any fight he was in.
Qel-Droma was one of the Republic's most capable Jedi during the events leading into the Great Sith War — sent to infiltrate the Krath, he went too deep, took the dark side on as a working tool, and never came back out. He ended up a Sith Lord in his own right, allied with Exar Kun, and led armies against the Order that trained him. He also killed his own brother.
When it ended, the Jedi didn't execute him. Nomi Sunrider severed his connection to the Force entirely — not a restriction or a suspension, but a permanent cut. For a Jedi, that's a more complete erasure than death: the lightsaber becomes an inert metal tube, the fighting forms become impossible, and the person is left with the memory of everything they used to be able to do.
He spent his remaining years in exile on the ice world of Rhen Var, and was eventually killed by a man who wanted the credit for killing a Sith Lord — shot by someone he could no longer sense coming.
